When praying for a sick person the custom is to preferably use their Hebrew name and their mother's Hebrew name.
In the boxes below, for each sick person please write their name in the following format
HEBREW NAME of the sick person, BAR (for a male) or BAT (for a female), HEBREW NAME of the mother of the sick person
